New Students
All New Student Health Forms are due by July 15
The State of Connecticut requires that we collect some basic medical information and immunization history for every student who attends classes at any of our campuses. You can do this by filling out the New Student Health Form, bringing it to your healthcare provider for completion, and uploading it and all supporting documents onto your Student Health Portal before classes begin.

Attending to these forms is your responsibility. If this information is not up to date in the health services databank, you will not be able to attend classes, will be blocked from future class registration and will be restricted from entry into University Housing.
Providing Immunization Records

- All students must upload proof of vaccination dates - either on a signed New Student Health Form or a printed vaccination record.
- Print out the New Student Health Form (Note: this form must be printed, hand-written and signed by a health care provider). Fill out the appropriate sections, then bring the form to your healthcare provider for completion.
- A physical is not required.
- Access your health portal by logging in with your SHU email address and password
- Select the Forms tab on the left hand side of the portal
- Select Immunization Form & Upload in the Forms tab. You will now input the dates of all required vaccinations using the information provided by your healthcare provider on your completed New Student Health Form or on your printed vaccine record. At the bottom of the page, you must upload your New Student Health Form and any supporting documents (for example, printed vaccination records or lab reports) by clicking on the Upload Documents button.
- Complete the High Risk Tuberculosis Screening Questionnaire in the Forms tab.
All students are responsible for ensuring that we have received all the required information. You should check the Messages tab on your portal within 5-10 business days after submitting your health forms to make sure that we are not missing any information. You should also check your SHU email for important emails from us.
Special Student Populations
Certain programs require their own health forms through private compliance companies such as CastleBranch, PreCheck, Complio or Exxat/Prism: Undergraduate Program: Nursing; Graduate Programs: Athletic Training, Nursing, Occupational Therapy, Physical Therapy, Physician Assistant & Speech-Language Pathology. Other academic programs may use these companies to check compliance as well; your program will notify you if you are required to submit these forms. This does not exempt you from submitting health forms to Student Health Services as required by the State of Connecticut.
Athletes
The health form requirements for Student Health Services are in addition to any health forms required by the Athletics Department. Even if you have submitted health forms to the athletics department, you must also submit the New Student Health Form to Health Services.
