If you got your diploma or certificate in the mail, and it later got lost or damaged, you may order a replacement using the link above. The replacement fee for each individual diploma or certificate is $80. You will receive your order in 9-10 weeks at the most. If you have any questions, graduation@sacredheart.edu. If you never got your diploma or certificate in the mail do not fill out this form. Instead, contact graduation@sacredheart.edu.

Change of Name Legal Procedure
Legal Name is the name shown on a person’s valid passport, US birth certificate or other United States or US state government issued identification unless subsequently modified by a United States or US state court document, such as a marriage certificate or valid court order.
- Email registrar@sacredheart.edu requesting a name change
- You will need to send the request from your Sacred Heart email account
- Include your ID number, a copy of the legal document of the name change and a photo ID with the new name
- A confirmation email will be sent once the name has been updated
Check the academic calendar for registration deadlines. Students are responsible for reviewing their class schedule in SPA to verify registration. Withdrawal policies and procedures can be found in the Academic Catalog under Academic Standards, Policies & Procedures.
Change of Chosen Name Procedure
Chosen Name is the name a person uses consistently and publicly to identify themselves. A person’s Chosen Name may be the same or different from their Legal Name. There are many reasons a person may use a Chosen Name different from their Legal Name including gender identity, a nickname, or a Westernized name, to name a few. SHU students, faculty and staff who would like SHU to use their Chosen Name must notify SHU according to the Procedures Supporting SHU’s Chosen Name Policy.
