Housing Accommodations for a Disability
The Housing Accommodations Committee (HAC) will evaluate accommodation requests for a student with a documented disability that substantially limits one or more major life activities.
Students seeking housing accommodations must provide documentation that clearly indicates the functional limitation(s) resulting from the disability that are addressed by the requested accommodation. This documentation must be provided by a clinical specialist qualified to diagnose and provide care for the student’s disability.
Request Housing Accommodations
- Complete the Housing Accommodation Request form
- Complete the Student Accessibility Intake form (if you previously submitted this form you do not need to submit it again)
- Provide documentation of your disability that meets the University’s documentation guidelines. Documentation must be uploaded with the intake form. The University reserves the right to request additional documentation. Do not send documentation through email. Student Accessibility will send you a link to a file transfer system in which you can securely upload your documents. - Student Accessibility will meet with each student who submits a timely and complete request, to engage in an interactive process. Use the OSA booking calendar to schedule your meeting.
You will be notified of HAC’s decision through your Sacred Heart University email.
