Chosen Name Change Form & Instructions

All classes starting prior to 11 a.m. will be remote. Classes after 11 a.m. will be in person. Non-essential staff should work remote until 11 a.m. then report to campus as scheduled.
Students, faculty and staff submit the Chosen Name Change Request Form to notify SHU of your Chosen Name.
Check your Student Planning & Advising (SPA) account before submitting this form. If you provided a “Preferred” name to SHU before September, 2023, that name will populate the Chosen Name field as SHU Systems are updated.
Students, faculty and staff provide their pronouns to SHU by the following steps:
- Log into your SPA account
- Choose “User Profile”
- Choose “Edit Personal Identity”
- There is a drop down menu to choose from a list of common pronouns. If you do not see the pronouns you use, contact Freda Grant at 203-396-8067 or grantf2@sacredheart.edu or the general equitycompliance@sacredheart.edu for assistance.
