1. Create an account on the CSDE website and follow the prompts to apply for certification, which includes a $50 charge that will go towards the total $200 application fee. This will begin the background check process with the CSDE while you wait for Sacred Heart to recommend you for certification. Once the CSDE reviews your electronic application, they will contact you to collect the remaining $150 balance.
  2. Submit to Jessica Beardsworth via email at beardsworthj@sacredheart.edu:
    • ED170A form filled out through section 1f on page 2.
    • Transcripts for any courses not taken at Sacred Heart that you have not yet submitted to Sacred Heart.
    • Certification test score reports only when you have passed them (Praxis Subject Assessments and Foundations of Reading, if applicable). Please also make sure you select Sacred Heart as a score recipient with ETS and/or Pearson.
  3. Once all final SHU grades have been posted to your official transcripts, mail the CSDE your official transcripts. The CSDE’s mailing address can be found at the top of the ED170A form. You will also need to send the CSDE your official transcripts from any other universities you have attended.
