Apply for your 097 certification on the CSDE website under your existing account. The application includes a $50 charge that will go towards the total $200 application fee. This will begin the background check process with the CSDE while you wait for Sacred Heart to recommend you for certification. Once the CSDE reviews your electronic application, they will contact you to collect the remaining $150 balance.
Once all final grades have been posted to your official SHU transcripts, please mail the CSDE your official transcripts to this address: P.O. Box 150471, Hartford, CT 06115-0471. Official transcript requests are processed at this location on our SHU website.
