Documentation should include an evaluation by an appropriate and certified professional. This evaluation should clearly identify the academic impacts of the disability as it relates to the accommodations and services that are being requested. Reports must include:

  • Name of the professional
  • Title and professional credentials, including licensing number
  • Date of the evaluation
  • Printed on professional letterhead
  • Typed
  • Dated
  • Signed by professional who completed the evaluation

The evaluation must include evidence of a disability including:

  • Date and or/age of onset of allergy and/or asthma
  • Information regarding presenting symptoms that significantly impact and restrict functioning
  • Discussion of the nature, frequency and severity of allergy/asthma symptoms, including prescription medication and frequency used
  • Identification of specific allergens (if any)
  • Identification of previous incidents and causes that have required immediate treatment (including hospitalizations)
  • Relevant background information such as: developmental, academic, psychosocial, family, etc…
  • If the student is requesting accommodated housing, discuss the specific impact of housing on the students condition
  • Identify prior accommodations for the student

Comprehensive Evaluative Summary:

  • Evaluator’s comprehensive explanation of provided scores for each assessment
  • Statement of diagnosis or specific impairment, including DSM-5 code (s)
  • Explanation of the type of functional limitations to major life activities created by the disability
  • List of current prescription medications and other treatments, including potential side effects and their frequencies. These include nebulizers, inhalers, shots, steroids, etc.
  • Specific accommodation, auxiliary aides and/or services recommendations for the academic and/or residential (if student is requesting housing) setting with evidence from the supplied evaluation
  • Identification of the evaluator including name, date evaluation was completed and evaluator’s signature

Students are encouraged to submit all required documentation along with their online request at least 6 weeks prior to the start of the semester.
