Before attending Sacred Heart University, every new student must complete and submit necessary medical paperwork to ensure a healthy experience.

The University requires your health care provider to complete health forms to comply with Connecticut State Law regarding student immunizations. Attending to these forms is your responsibility. If this information is not up to date in the health services data bank, you will be blocked from future registration and you will be restricted from entry into University housing.

Forms

New Student Health Form (Note: this form must be printed, hand-written and signed by a health care provider)

  • Required for all new students.
  • Due on July 15 for fall semester start date (or four weeks prior to start of classes if spring/summer start)
  • Student Athletes: The New Student Health Form is separate from the Division 1 forms or Club Sports forms that are required by the Athletics Department. All new students must submit the New Student Health Form to Student Health Services as required by the State of Connecticut.
  • Certain programs require their own health forms through private compliance companies such as CastleBranch, PreCheck, Complio or Exxat/Prism (Undergraduate Program: Nursing; Graduate Programs: Athletic Training, Nursing, Occupational Therapy, Physical Therapy, Physician Assistant & Speech-Language Pathology). Other academic programs may use these companies to check compliance as well; your program will notify you if you are required to submit these forms. This does not exempt you from submitting health forms to Student Health Services as required by the State of Connecticut.

  • Only for students who have positive TB screening tests. You will be notified if you are required to fill out this form.

Vaccine Exemption Request Forms

Medical Exemption to Vaccine Requests

Medical exemption request forms should be completed by your healthcare provider (MD, DO, PA or APRN only). We can only accept requests made on the official state of CT "Student Medical Exemption Certificate for Required Immunizations" form linked below. Students must fill out the Medical Exemption Waiver Form linked below. Both forms, once completed, must then be uploaded to your Student Health Portal.

Medical Exemption requests will be reviewed by the SHU medical advisory committee once both forms are received and are granted at the sole discretion of the committee. You will be notified through your Health Portal whether your medical exemption has been granted, usually in about 1-2 weeks.

Religious Exemption to Vaccine Requests

As of April 28, 2021, Connecticut no longer allows religious exemptions to state-mandated vaccines in colleges and universities. Students only qualify for a religious exemption to the state-mandated vaccines if they were enrolled at Sacred Heart University and had a valid religious exemption form on file in our office prior to this date.
