Once students are formally accepted into the graduate program in Occupational Therapy at Sacred Heart and have deposited, the following are required in order to enroll and begin classes:
By June 15: Signed Essential Functions Policy agreement. Students must be able to meet the requirements for the essential functions consistent with occupational therapy practice with or without reasonable accommodations. A link to the agreement will be emailed to you from Graduate Admissions.
By June 15: Registration for the fall trimester (registration forms will be emailed to you from the OT program). Send completed registration forms to the Occupational Therapy program June 15.
By August 1: Successful completion of a Criminal Background Check. The College of Health Profession’s Criminal Background policy and instructions will be emailed out to you from the OT program. You may view the policy at Criminal Background Check Policy. Criminal Background Check must be completed by August 1.
By August 1: Completed health requirements (instructions will be emailed to you from the OT program) uploaded to your Exxat portal by August 1. Please note that after uploading health forms to Exxat, an additional copy of the Exxat forms must be uploaded to University Health Services Student Health Portal to meet the State of Connecticut requirements for college admission.
- Proof of American Heart Association BLS for Health Care Provider (CPR & AED) uploaded to your Exxat portal.
- Proof of Health Insurance uploaded to your Exxat portal.
By August 1: Information Release Authorization Form (this form will be emailed to you from the OT program). Return to the Academic Department Assistant by August 1.
By August 1: Entering Student Data Form (this form will be emailed to you from the OT program). Return to the Academic Department Assistant by August 1.
By August 1: Successful completion of OSHA/HIPAA training course in your Exxat portal.
By August 21: Signed Photographic Consent Form. This form will be emailed to you from the OT program. You may decline but you will need to accept responsibility to avoid appearing in pictures that will be posted on the web. Return (if applicable) to the Academic Department Assistant by August 21.
By December 31 of first fall trimester: Successful completion of VIRTUS training. Sacred Heart University and our Diocesan-Affiliated fieldwork sites require VIRTUS training for all students, faculty, staff that may have contact with children. Protecting God's Children® for Adults is training conducted by VIRTUS® certified facilitators on the prevention of child sexual abuse. The training makes participants aware of the signs of child sexual abuse, the methods and means by which offenders commit abuse, and steps one can use to prevent child sexual abuse. Occupational Therapy students must complete this three-hour training session and provide a copy of your certificate of training to the Academic Department Assistant by the end of the December of the first trimester of the program. Completion of any outstanding prerequisites with a grade of C or better and verifying continued maintenance of all GPA and grade admission requirements. Send final official transcripts to Sacred Heart University Office of Graduate Admissions prior to enrollment.
If applicable, official transcript verifying completion of undergraduate bachelor’s degree.
Students are required to have a laptop computer, printer and reliable internet access. Sacred Heart University supports student success by providing free access to the Microsoft Office Professional Suite, Blackboard, Zoom and extensive library databases, e-books and journals, along with 24-hour technical support through the University’s IT Help Desk.
The Occupational Therapy (OT) program further enhances learning through access to specialized instructional technologies and simulation resources, including iPads, classroom video technology, patient simulation technology, Driver Simulation, Safe Patient Handling technology, the Anatomage Table for digital anatomy simulation and the Human Anatomy Lab (HAL). These learning environments are maintained and supervised by OT faculty and trained technical staff to ensure safe, accurate and effective instructional use.
The graduate program in Occupational Therapy is housed within the College of Health Professions located at the Center for Healthcare Education, 4000 Park Avenue, Bridgeport, CT.
