Club Reactivation Form

This form can be used to apply to reactivate a club that has not been active for a semester to two years. Any organization inactive for more than two years must reapply the club as a new club.

By reactivating this club you are agreeing to the Council of Clubs and Organizations (CCO) Guidelines. You agree to:

  • Plan and host weekly or bi-weekly club meetings
  • Plan and host campus wide events
  • Attend Open Houses and CCO President’s meetings

To reactivate a club you need:

  • 5 Full Time Undergraduate Executive Board Members
  • At least 5 Full Time Undergraduate General (additional) Members
  • A Statement of Purpose: stating why you want to reactive this club
  • Full-time Faculty/Staff Advisor
