Below you will find forms and documents that will assist you in being a successful club on campus. Some forms are required to be turned in by certain deadlines each semester, so be sure to check your emails and attend President and/or Advisor Meetings!

Important Dates to Remember

My club/organization will:

  • Submit the Event Registration Form (ERF) to reserve classroom space and time for club meetings by September 9, 2016
  • Have First Club Meeting before September 16, 2016 to occur either on a weekly or bi-weekly basis

My club/organization will submit:

  • Complete Club Roster form to CCO no later than September 24, 2016 (Advisors to enter online by September 30, 2016)
  • End of Semester Report by December 12, 2016
  • Spring 2017 Registration Form by December 12, 2016

I, as acting club/organization President will:

  • Attend monthly President’s meetings held by CCO
    • Dates to be determined at the beginning of each academic semester
  • Attend and/or arrange for representation from my club/organization at Open Houses throughout the academic semesters
    • Dates to be determined by Admissions
