    Grants

    • Co-Chair for Population Health Workgroup of Connecticut Nursing Collaborative-Action Coalition - Development of an educational program on Population Health based on evidence in the literature. To be available to all nurses free on the CLN website soon. Project funded by the RWJ – SIP grant.
    • Building Partnership between ESDHD and Local Large Health Care System - Pilot/foundation project with GHCT, NEMG, and East Shore District Health Department. Through data sharing and potential funding, target populations in district with chronic illness and develop interventions and programs.
    • Community Health Centers Inc. - DEU partnership. Our goal is to expose students to the primary care system that cares for the underserved populations by focusing on improving health outcomes. My role is to provide direct evaluation support, collaboration in student coordination, refining DEU competencies to evolve with QU curriculum, IPE coordination support, data collection of post graduate information. Assist with an enhance across-system care coordination learning with an innovative approach called Extension for Community Healthcare Outcomes model (ECHO). Project funded by the HRSA (federal) grant.
