Welcome to Sacred Heart University! Student Health Services will be an integral part of your University education and experience. In order that we may better serve your health needs, the following requirements should be noted and adhered to:
I. HEALTH FORM
An individual health record for each student will be maintained in a confidential file. The university requires an accurate and current health record from each student before admission. This helps to protect the health of the student community. A physical exam is not required except for athletics/sports participation. The athletic department requires and maintains separate records.
NOTE: Part of the health history is a record of immunizations. The State of Connecticut and the University require proof of immunization to measles (rubeola) and German measles (rubella). A TB test (given within six months of start of classes) and a tetanus vaccine (given within the last nine years) are required. A meningitis vaccine is required for any student living in university housing. Hepatitis B vaccines are strongly recommended for all college students.
II. MINOR CONSENT FORM
Parents of students under the age of 18 must complete and sign a “Consent For Treatment of Clients” form before students under the legal age can be treated in Health Services.
All forms must be completed as soon as possible and returned to Health Services, Sacred Heart University, 5151 Park Avenue, Fairfield, CT 06825. These forms must be returned before August 1, 2008.
