Purpose: This service provides staff and faculty the ability to dial up to the internet from off-campus in the absence of an available broadband connection.

Instructions: To use Dial-Up provided by SHU a Dial-Up account must be setup. To request one, please have your account manager fill out the information on this form as an authorization for this service ($20 per month fee) and fax to the number at bottom of form. Please allow one week for processing.
