PROCEDURES TO FOLLOW WHEN YOU HAVE A WORK RELATED INJURY OR ILLNESS• It is very important that you report any work related injury or illness immediately, whether or not you need medical treatment right away. A record/report of your injury is required for workers' compensation insurance coverage and is really important in case you need to seek treatment later.
• Please be sure to follow the protocol established for treatment of work related injuries. Initial treatment for a work related injury/ illness must be obtained at St. Vincent's Immediate Healthcare (IHC) in Bridgeport, CT. The doctors at St. Vincent's IHC will refer you for specialist care if needed. Claims for continuing care received at a facility or doctor's office that is not a participating provider in the CareSys Managed Care Network (workers' comp insurance network) may not be covered.

1. REPORT THE INCIDENT TO YOUR SUPERVISOR AT THE TIME OF INJURY / ILLNESS
2. SEEK TREATMENT AS SOON AS POSSIBLE BY VISITING THE ST. VINCENT'S IMMEDIATE HEALTHCARE CENTER LOCATED AT:

For All After-Hour Emergency Care, Please Visit the Nearest Emergency Room Or Other Outpatient Facility.
3. AS SOON AS POSSIBLE, VISIT OR CALL THE PUBLIC SAFETY OFFICE (203-371-7995) TO COMPLETE A FIRST REPORT OF INJURY FORM AND ALSO CALL THE EMPLOYEE BENEFITS OFFICE AT 203-371-7756 OR 203-371-7921 FOR FURTHER INFORMATION ABOUT COVERAGE, NEXT STEPS, ETC. (IN CASE OF SERIOUS INJURY, THE SUPERVISOR SHOULD INFORM PUBLIC SAFETY AND EMPLOYEE BENEFITS OFFICE AS SOON AS POSSIBLE).
4. SUBMIT ALL PAPERWORK / BILLS FOR CARE RECEIVED TO THE BENEFITS OFFICE AS SOON AS YOU GET THEM.
