TEST REQUEST FORM: Click here to download a copy of the test request form. Fill out the form and submit the form in person to LaDonna Reynolds or Jim Malerba in the Office of Special Services at least 72 hours (not including weekends) prior to any exam and/or quiz. We do not accept test requests via e-mail.
NOTE: To guarantee the integrity of the test-taking process, the Office of Special Services needs to restrict its testing availability to approximately 45 hrs per week so that the JLC can be properly staffed for proctoring at those times We ask that when a student with a documented disability or a member of the Office of Special Services contacts you regarding an extended time request, that you do your best to schedule the test within the above time frame.
ONE FINAL NOTE: During final exam week, the Office of Special Service staff suspends the above traditional proctoring hours and monitors students with disabilities until all tests are completed on all exam days—even if this means proctoring until 10 pm.
