- Download the New Account Request Form. Form can be created in Excel, but must be printed out and sent via mail.
- All fields must be completed
- Attach copies of any pertinent documentation
- Please print the list of object codes and circle/highlight the expenses you think you will need & send with your account request
- ALL account requests MUST BE APPROVED by one of the following positions or higher:
- Assistant Vice President
- Dean
- Executive Director
