Verification Forms

NEW: Beginning with the 2017-18 FAFSA, families are required to report income information from an earlier tax year. For example, on the 2017-18 FAFSA, students (and parents, as appropriate) have to report their 2015 income information, rather than their 2016 income information.

If your FAFSA is selected for federal verification you must submit additional documents (located under the My Documents tab on SHUAwards) to our office.; a verification worksheet, provide IRS Income Verification from 2015, along with 2015 W2s (non-tax-filers only).

Tax Filer Directions

IRS INCOME VERIFICATION– 2015 TAX YEAR:

For tax filers, the most efficient way to provide this information is through the use of the IRS Data Retrieval Tool (DRT) on the FAFSA. Tax filers may instead opt to request a tax return transcript through the IRS. Due to the difference in processing time, we strongly recommend utilizing the IRS Data Retrieval Tool. 

Please note recently widowed, separated, married - filed separately, or divorced parents that were required to file a joint return are NOT eligible to use the IRS Date Retrieval Tool. In these cases you will need to request the tax return transcript(s). 

Option 1 – IRS Data Retrieval Into FAFSA:

If the student/parent has not already used the tool, go to FAFSA.gov, log in to the student’s FAFSA record, select Make FAFSA Corrections and navigate to the Financial Information section. From there, follow the instructions to determine if the student/parent is eligible to use the IRS Data Retrieval Tool to transfer 2015 IRS income tax information into the student’s FAFSA. Once the IRS data transfer is complete, electronically SIGN and SUBMIT your FAFSA.

Please note that it takes up to two weeks for electronic tax return filers and up to eight weeks for paper tax return filers for IRS income information to become available for the IRS Data Retrieval Tool. 

IRA Rollers and Pension Rollovers with IRS DRT: The IRS DRT does not exclude tax return IRA and/or Pension Distribution Rollovers from untaxed income. This means the IRS DRT will include an IRA and/or Pension Distribution Rollover in your untaxed income on the FAFSA. This may result in a higher Expected Family Contribution (EFC) and decrease in your financial need. Do not attempt to correct the information yourself. By changing any of the tax data, you compromise the data transferred from the IRS. Provide the following to if you do have an Untaxed IRA and/or Pension Rollover:

  • A written notice/request for correction to your FAFSA Untaxed IRA and/or Pension Rollover;
  • A copy of Page 1 of your Federal 2015 IRS Federal Income Tax Return;
  • A copy of IRS Form 1099-R for all IRA or Pension distributions.

Option 2 - IRS Tax Return Transcripts:

If you do not or cannot use the IRS Data Retrieval Tool, all tax filers must submit a 2015 IRS Tax Return Transcript; you may obtain this by one of the following:

  • Online Request: Follow this link and choose Get Transcript Online or Get Transcript by Mail. Be sure to request the IRS Tax Return Transcript and NOT the IRS Tax Account Transcript

  • IRS2Go App:  You may download the app at either of the following to request a transcript:
  • Telephone Request: 1-800-908-9946
  • Paper Request Form: IRS Form 4506T-EZ or IRS Form 4506-T

Special Circumstances Directions

IRS INCOME VERIFICATION– 2015 TAX YEAR:

Tax Extension

  • Form 4868 that was filed with the IRS for the tax year
  • Copy of the IRS approval of the extension beyond the 6-month extension (if applicable)
  • 2015 W-2 Form(s) or if self-employed, a signed statement certifying the 2015 AGI and Income Tax Paid
  • Verification of Non-filing Letter (confirmation that the tax return has not yet been filed) from the IRS or other relevant tax authority dated on or after October 1, 2016. NEW REQUIREMENT in 2017-18 

Identity Theft

  • Tax Return Database View (TRDV) Transcript obtained from the IRS by calling 1-800-908-4490
  • Statement signed and dated by the tax filer indicating that they were victims of IRS tax-related identity theft and that the IRS has been made of aware of the tax-related identity theft

Amended Returns

  • IRS Form 1040X (signed)
  • IRS Tax Return Transcript that includes information from the original OR any other IRS Transcript that includes all income and tax information from the original tax return required for verification

Foreign Tax Returns/Puerto Rico, Northern Mariana Islands, Guam, American Samoa, or U.S. Virgin Islands Tax Returns

  • Transcript from relevant taxing authority listing tax account information for the tax year, if transcript can be obtained at no cost. Otherwise, a copy of tax return filed with taxing authority.

Non-Tax Filer Directions| New Requirement for Non-Tax Filers in 2017-2018

IRS INCOME VERIFICATION– 2015 TAX YEAR:

IRS Verification for non-tax filers can be obtained by requesting an IRS Verification of Non-filing Letter from the IRS. This letter provides proof that the IRS has no record of a filed Form 1040, 1040A, or 1040EX for the 2015 tax year. Please also submit copies of your 2015 W2s if you did not and were not required to file a tax return in 2015.

IRS VERIFICATION OF NON-FILING LETTER – 2015 TAX YEAR:
Non-Tax filers can request an IRS Verification of Non-filing of their 2015 tax return status by using the following:

  • Online Request: Follow this link and choose and choose Get Transcript Online or Get Transcript by Mail. Make sure to request the "Verification of Non-filing Letter" Tax Year "2015"
  • IRS2Go App: download the app at the following to request a transcript of IRS Verification of Non-filing Letter
  • Telephone Request:1-800-908-9946 - Select "Option 2" to request an IRS Verification of Non-filing Letter and then enter "2015".
  • Paper Request Form: IRS Form 4506T-EZ or IRS Form 4506-T.

Sign and submit the IRS Verification of Non-filing Letter to Sacred Heart University; make sure to include the student’s name and Sacred Heart ID number on the letter.

Submit All 2015 W-2 forms If the student/parent will not file and is not required to file a 2015 income tax return with the IRS, we will require you to submit copies of all of your 2015 W-2 Forms.

A Social Security Number, an Individual Taxpayer Identification Number, or an Employer Identification Number is required to obtain a verification of non-filing from the IRS. If you fall into this category, please be sure to complete the Income Verification section on the Verification Form (if applicable).
