Clinical Site Quality Evaluation Form

Only AT Core Faculty members will conduct clinical site evaluations. Site visits will occur a minimum of one time per academic year. All clinical sites will have 100% compliance with all items on the Quality Checklist Site Evaluation form.

Student Evaluation of Professors

Evaluation form is completed by student assessing the program courses. For the objective to be met, average score is to equal a 3 or higher.

100% of faculty earned a 3.5 or higher on “Quality of Instruction” for each of their courses.

Student Evaluation of Preceptors

  • Objective 3

Evaluation form is completed by student assessing their preceptor and clinical rotational experience. For the objective to be met, average score is to equal 3 or higher.

40 preceptors were evaluated in the 16/17 academic year. 1 preceptor received a 2 on one portion of the evaluation.

The evaluation was discussed with the preceptor’s supervisors as well as with the preceptor. The plan includes bi-weekly discussions with the preceptor through the fall semester and the preceptor’s students. If an issue arises again, the preceptor will be put on probation for 1 semester and students will be removed from the site. The preceptor will have to complete specialized training geared towards rectifying the situation.
